IMPORTANT ANNOUNCEMENT - MUST READ

To all friends who have placed their orders from 29th February 2012 onwards you really need to fill in the form below. To those who have done it, thank you so much :)
For your information, we are migrating to a new order tracking software and we seek your cooperation to submit the form to ensure all orders are well recorded.
We do have one good news :) and we would like to share this with you now.
My sister has agreed to help distribute your orders while I'm away in April 2012. However, she will rely on these forms (the one I'm asking you to fill up and submit) to trace your order, confirm your address, communicate with you if there's a need to and POSLAJU them to you. If you do not submit the form, you will have to wait till I'm back in May to do that.
Instruction :
(1) Please use one form for one order. If you order 3 bags at one time and they were paid together, consider it as one order.
(2) Please upload the picture of the item you ordered (my sis is new to this business and she needs picture to identify the bags). You may use word file or jpeg etc but please make sure you upload something :)
(3) Please mention the full name, brand and also the style number of the bags/wristlet you ordered (again, to make things easier for my sister!)
